Comments: Please ReadRN Coordinator- Patient Transfer Call Center (similar to Telephonic RN) - THIS IS A NIGHT SHIFT ASSIGNMENTDescriptionPlans- coordinates and screens for clinical appropriateness for inpatient transfers. Responsible for transfer process and adherence to requirements including State and Federal regulations. Collaborates with physicians- hospital staff- insurance companies- and patient-family. Coordinates and acts as liaison to facilitate and manage appropriate and timely health care services. In the Queens Transfer Center- coordinates acceptance or denial of inter-facility transfers with physicians based on clinical need and Medical Center resources. In collaboration with Bed Control- reviews direct and emergency admissions for clinical and financial appropriateness and assists with bed coordination and census management. Performs all admission screenings and documentation for clinical appropriateness using evidence based guidelines. RequirementsMust work 40 hrs-week- combination of 8
